When enrolling a new student, the following documents must be received from the parent or guardian before registration can be finalized or placement offered:

• BOLCA Student Online Enrollment Application 
• Student's Birth Certificate- Original
  
• Current Immunization- Original
  
• School Entry Health Exam- Original
• Report Cards- (The last 2 years)
  
• Withdrawal Form- Previous School
• Standardized Testing (The last 2 years) 
• Copy of Psychological Evaluation (McKay or Special Needs Learners)
• Copy of most recent of  (IEP) Individualized Education Plan (McKay or Special Needs Learners)
• End-Of-Year Transcripts with Attendance
• Proof Of Residency
  
• Parent(s) Identification and/or Guardianship Documentation 
• Signed Photograph Release Form
